2 theories.

- Fetishists of sh*t exist. I imagine people fascinated with horrid things could be attracted by Nurgle.

- i can imagine nurgle servants spreading some horrible plague in cities of villages, promising release from the pain to anyone that will follow the only one that can save them.

Also you have the first part of Grandfather's gifts, toughness. In a world of conflict being able to thrive despite injury is huge. The big N is also about life, so when you're an obsessed apothecary trying to figure out how to save people... here come Papa to teach you the secrets. Sure your body, soul and mind slowly turn to rot but the knowledge you sought is yours. Bad crops due to blight? Might want to sacrifice to try and get past it.
